  2. Clinician agrees to keep Supervisor informed about all aspects of their clinical work and to implement all supervisory directives. If, after a course of action has been agreed to, additional information becomes available or changes in circumstances cause Clinician to believe that reconsideration is warranted, they will revisit the issue with Supervisor in a timely manner.

  3. Clinician will ensure to consult with Supervisor about the following situations:

    • disputes with clients or impasses in the therapy;

    • allegations of unethical behavior by clients, colleagues, or others (e.g., a client’s family members);

    • threats of a complaint or lawsuit;

    • mental health emergencies requiring immediate action;

    • high-risk situations; cases in which clients evidence suicidal thoughts, gestures, or attempts or a significant history of attempts; or cases in which clients present with a history of, propensity for, or threats of violence;

    • contemplated departures from standards of practice or exceptions to general rules, standards, policies, or practices;

    • suspected or known clinical or ethical errors and related countertransference;

    • contact with clients outside the context of treatment;

    • legal issues, such as possible reporting obligations related to suspected abuse of a child or vulnerable adult or ethical violations by other professionals.

  4. Supervisor is responsible for ensuring documentation of clinical supervision sessions meets the Board's requirements in A.A.C. R4-6-212.

    • Supervisor will maintain records of all supervision sessions for at least seven (7) years.

    • Supervisor will provide feedback on clinical work and evaluate performance, documented in clinical supervision notes.

    • The Board may request documentation of Behavioral Health Examiners. Supervisor will inform Clinician of requested releases from the board.

  5. Supervisor will provide at least one hour of monthly clinical supervision. Clinician may request supervision anytime, and Supervisor will respond to requests within 24 hours.

  6. Supervisor will maintain compliance with educational requirements for clinical supervision and maintain his listing on the Clinical Supervisor Registry with the Board of Behavioral Health Examiners.

  7. Clinician’s clients will be notified of Supervisor’s involvement in their treatment and the means to contact them. Clinician’s clients will authorize the release of their clinical records to Supervisor.

  8. Termination of the supervisory relationship.

    • The supervisory relationship may be terminated for the following reasons:

      • noncompliance with supervisory directives.

      • concealment or misrepresentation of relevant information about themselves or clients.

      • violations of ethical standards or laws.

      • frequent tardiness or absences.

      • inability to practice with reasonable skill and safety.

    • Supervisor will provide adequate notice and describe specific reasons, in writing, for termination.

    • If the supervisory relationship is prematurely terminated, Supervisor will support Clinician in their transition.
